To register for one of our seminars, please do the following:

• Print the form and fill it out
  
  
• A deposit of $250.00 is due with your completed registration form.
  
  
• For those paying by check or money order, mail the form along with the deposit to address at the bottom of the form.
  
  
• For those paying by credit card, either mail the form to address at the bottom of the form or fax the form to the fax number at the bottom of the form.
